From the initial tour of the facility and completion of the intake package through discharge planning, on-going supportive visits, and compassionate presence during end of life care, the social services staff at Fox Subacute provides supportive and concrete services to both the residents and their families.
Specifically, the social services worker(s) assist with the transition from the acute care setting to the nursing facility through regular supportive visits, scheduled care conferences and generally being available to both the resident and the family as needs arise. The Social Worker assists the family/resident in formulating questions as well as directing those questions to the appropriate staff. Separate family meetings are also facilitated as a support for the family members who are also coming to grips with the reality of their loved one’s medical situation.
The social services staff at Fox Subacute coordinates discharge planning for all residents both for transfers to other facilities as well as moving back to the community; this includes arranging for in-home nursing or aide care, DMEs, respiratory equipment, infusion care, etc.
Fox Subacute’s facilities have a low number of beds, this allows our social workers the opportunity to get to know the families and residents, those here only for a short time as well as those here for long-term care. Together in working with the entire treatment team, meeting the individual needs of the resident’s medical, physical, social and emotional care, are our paramount concern.
